Case Manager II*
Position # 209
Responsible for providing case management for seriously mentally ill clients. Establish and maintain therapeutic relationship; max clients access to services and ability to integrate into the community post hospitalization. Will also provide provision of direct service support to the clients natural support system.
Responsible for development and implementation of a treatment plan
and completion of other documentation.
Case Manager IV***
Position # 204
Responsible for establishing and maintaining the ongoing
therapeutic relationship; and carry caseload of 20 mixed risk level
clients. Provision of intensive community-based services to maximize
the client’s access to services and ability to function adequately
and integrate in the community. Responsible for assisting team
leader in
supervision, report preparation and meeting participation.
Development and implementation of a treatment plan and completion of
other documentation as required. Monitoring of service delivery to
meet a client's changing needs and advocacy as necessary. Liaison
with psychiatric, inpatient units to effectuate coordinated and well
planned hospital discharges. Record keeping as required by funding
sources, and/ or accreditation agencies, including case notes, group
therapy notes, treatment notes, treatment planning, termination
summaries, correspondence with insurance providers, outcome data.
Prospect House
Case Manager - BA**
Position # 21
Responsible for daily implementation of Partial Care Program, in
collaboration with staff, supervisors and members, to provide partial care
services to individuals with severe and persistent mental illness, to
enhance community functioning. Plan and facilitate therapy, coping, activity
and psycho-educational groups to enhance
social skills and promote relapse intervention and independent living.
Provide case management services for members as assigned. electronic
clinical records including case notes, treatment planning, outcome data and
other reports as required by funding sources.
GENERAL REQUIREMENTS:
*Candidates must have a BA degree in psychology, social work or
related discipline +a minimum of 2/yrs experience in the mental health
field.
**Candidate must have a Bachelor’s degree in human services field, or an
Associate’s degree and two years experience in providing human services.
Computer literacy required. The ability to drive a van is required.
***Candidates must have a master’s degree in psychology, social work,
counseling or a related field with clinical training or actively taking
coursework towards earning a Master’s Degree in aforementioned field.
Able to work a flexible schedule, including evenings, weekend and
holiday hours.
